This checklist should be completed when a manager or supervisor suspects drug or alcohol use based on the physcial appearance and behavior of the employee. Use it to appropriately determine drug or alcohol abuse at work and to document the reasonable suspicion of an employee influence of drugs/alcohol prior to sending them to get tested.
